San Raffaele International MD 2027: what's new for 2027/28
The 2027/28 International MD call is expected mid-December 2026. The 2026/27 test: 60 questions in 80 minutes, 150 places, two sessions from home.
Students preparing for the International MD Program at Università Vita-Salute San Raffaele all ask the same thing in autumn: is the 2027/28 call out, and has anything changed? As of 24 October 2026, the call for 2027/28 has not been published. UniSR usually publishes it in mid-December, on its online notice board.
Until then, the best reference is the 2026/27 call. Below we sum up what it said, what we will check first when the new call comes out, and how to prepare in the meantime. Keep in mind that this is a different test from the Italian-language Medicine and Surgery test at San Raffaele, which has its own call.
Is the 2027/28 call out?
Not yet. The call is expected in mid-December 2026 and will be posted on the UniSR Albo online and linked from the International MD admissions page. In 2026/27 there were two separate calls, one for EU candidates (DR 9853) and one for non-EU candidates (DR 9854), both dated 15 December 2025.
As soon as the 2027/28 call is out, this guide will be updated with the new dates and any changes.

Key dates in 2026/27
These are last year's dates, useful to get a sense of the timing. They are not the 2027/28 dates.
- Calls published: 15 December 2025
- Applications, EU candidates: 17 December 2025 to 6 February 2026
- Applications, non-EU candidates: until 5 February 2026
- Test, EU candidates: 14 February and 14 March 2026
- Test, non-EU candidates: 11 February and 4 March 2026
Rankings usually come out between March and June. If the 2027/28 calendar follows a similar pattern, the first session would again fall in February, which leaves only a few weeks between the call and the test. That is why it pays to prepare before the call is out.

What we will check first in the new call
When the 2027/28 call is published, these are the points to compare with 2026/27:
- Structure: whether it stays at 36 + 24 questions in 80 minutes.
- Scoring: whether +1 / -0.25 / 0 is confirmed.
- Sessions: whether there are still two, both from home, with the better score counting.
- Dates: application window and the two test days, separately for EU and non-EU candidates.
- Places: whether the 86 EU and 64 non-EU places change.
- Fee: whether it stays at €200 for both sessions.
A change worth knowing: the Italian test is now different
In 2026/27 the Italian-language Medicine and Surgery test at San Raffaele changed to 20 biology, 20 chemistry and 20 physics questions in 60 minutes, with logic and text comprehension removed. The International MD test did not follow: in 2026/27 it kept 36 questions on logic, problem solving and comprehension and 24 on science, in 80 minutes.
So if you find practice material online labelled "San Raffaele", check which test it is for. Material for the Italian test no longer matches the International MD format.
What did not change: no past papers
UniSR still does not publish past papers for the International MD test, and there is no detailed syllabus beyond article 5 of the call. The questions come from a UniSR question bank, drawn at random by the external provider. UniSR sells its own simulator, with a free 20-question demo.
How much competition to expect
There is no fixed pass mark: your place depends on your position in the ranking, and EU and non-EU candidates are ranked separately. In 2026/27, 360 EU and 391 non-EU candidates appeared in the rankings. Position 86 on the EU list had 41.00 points; position 64 on the non-EU list had 48.75 points.
What this means for your preparation
You do not need to wait for the call to start. Logic, problem solving and comprehension made up 36 of the 60 questions in 2026/27, so that block deserves most of your time, followed by the four science subjects. Practise under the real conditions (80 minutes, negative marking, everything in English) and track your score over time. You can start now with TestBuddy's simulations for the San Raffaele International MD, then adjust once the 2027/28 call confirms the format.
