San Raffaele International MD 2027: complete guide to the admission test
San Raffaele International MD test explained: 60 questions in English in 80 minutes, scoring, two online sessions, places and what to expect for 2027/28.
Anyone looking at the International MD Program at Università Vita-Salute San Raffaele in Milan ends up asking the same things. What the test looks like, when it takes place, how it is scored, how many places there are and how hard it is to get in.
Here is the short answer. It is a 60-question test entirely in English, 80 minutes long, taken online from home in two sessions, and only your best result counts. The 2027/28 call has not been published yet (as of 24 October 2026), so the figures below are from 2026/27 unless stated otherwise.
What the San Raffaele International MD test is
It is the entrance exam for the single-cycle degree in Medicine and Surgery taught in English at Università Vita-Salute San Raffaele (UniSR), known as the International MD Program.
The course has limited places and admission depends on your position in the ranking, which is built on the test score.
It is not a translation of the Italian test: UniSR also runs a separate Medicine and Surgery admission test in Italian, with its own call and a different structure, so make sure you register and prepare for the right one.
What we know about 2027
The call for 2027/28 is not out yet. In recent years UniSR has published it around mid-December, and the 2026/27 calls (DR 9853 for EU candidates and DR 9854 for non-EU candidates) are dated 15 December 2025.
Until the new call appears, the 2026/27 rules are the best guide. Dates, fee and number of places can change, so check the official call as soon as it is published.
Structure of the test
| 2026/27 | |
|---|---|
| Questions | 60, all in English |
| Logic, problem solving and reading comprehension | 36 |
| Science (biology, chemistry, mathematics, physics) | 24 |
| Duration | 80 minutes |
| Reserve questions | 7, after the time is up, not counted in the score |
| Format | Online from home, with remote proctoring |
The call groups the 24 science questions together and does not say how many come from each subject. Any per-subject breakdown you see elsewhere is an estimate, not an official figure.
UniSR does not publish a detailed syllabus for this test: the reference is article 5 of the call. More detail is in our guide to the structure and syllabus.
Scoring, ranking and results
| Answer | Points |
|---|---|
| Correct | +1 |
| Wrong | -0.25 |
| Blank | 0 |
The maximum score is 60 points.
There is no fixed pass mark. What matters is your position in the ranking compared with the number of places. EU and non-EU candidates are ranked separately.
To give an idea of the level, in the 2026/27 final rankings:
- 360 EU candidates and 391 non-EU candidates were ranked
- the EU candidate in position 86 (the last place in the EU quota) had 41.00 points
- the non-EU candidate in position 64 (the last place in the non-EU quota) had 48.75 points
These are 2026/27 results and will not be the same in 2027. In past years the rankings have come out between March and June.

Places available
For 2026/27 there were 150 places: 86 for EU candidates and 64 for non-EU candidates. The number for 2027/28 will be in the new call.
Two sessions, best score counts
The test is offered in two sessions and your better score is the one used for the ranking. The test and the remote proctoring are run by an external provider, Selexi S.r.l. The questions belong to UniSR and are drawn at random from a question bank.
The 2026/27 dates were:
| EU candidates | Non-EU candidates | |
|---|---|---|
| Registration | 17 December 2025 to 6 February 2026 | 17 December 2025 to 5 February 2026 |
| First session | 14 February 2026 | 11 February 2026 |
| Second session | 14 March 2026 | 4 March 2026 |
The 2027 dates have not been announced. If the calendar follows last year's pattern, registration will open in December and the sessions will fall in February and March, but treat this as a pattern, not a confirmed date.
How much the test costs
In 2026/27 the application fee was €200, covering both sessions. The 2027/28 fee will be confirmed in the new call.
Past papers and official practice
UniSR has not published past papers for this test. It sells its own online simulator, with a free 20-question demo and paid full simulations (prices on unisr.it).
Since there are no official past papers, the best way to get used to the format is to practise with full-length, timed tests that use the same scoring, so that you learn to handle the -0.25 penalty and the 80-minute limit.
What this guide cannot confirm yet
One requirement is already clear from the 2026/27 call: to enrol you need an official English certificate at level C1 of the CEFR. It is needed to enrol, not to sit the test, but it takes time to obtain, so plan for it early.
Some details depend on the 2027/28 call and are not available yet:
- the exact test dates and registration deadlines
- the fee and the number of places
- eligibility requirements, accommodations for disabilities or learning disorders, and procedures for non-EU applicants
Check each of these in the call once UniSR publishes it.
How to prepare
Start from the official structure: 36 questions of logic, problem solving and reading comprehension, and 24 of science. The first block is the larger one, so it deserves steady practice from the start, not only in the last weeks. Then build up the four science subjects and train under real conditions: 60 questions, 80 minutes, negative marking.
You can practise with the TestBuddy simulator for the San Raffaele International MD test, which reproduces the timing and scoring of the official test. Our guide on how to prepare has a full study plan.
