If you are aiming for the International MD Program at Università Vita-Salute San Raffaele in Milan, the first practical question is simple: where do you find real practice material? The short answer is that UniSR does not publish past papers for this test, so you have to build your practice around the official format described in the call for admission.

This guide explains what exists, what does not, and how to train in a way that matches the real exam. The call for 2027/28 is not out yet (it usually comes out in mid-December 2026), so every figure below refers to the 2026/27 call unless we say otherwise. One more thing before we start: this is a different exam from the [Italian-language Medicine and Surgery test at San Raffaele](https://testbuddy.it/test-universita-private/san-raffaele/guida-completa), which has its own call and its own structure.

## Are there official past papers?

No. For the International MD test, UniSR has not published any past papers. The questions belong to the University and sit in a question bank from which the external provider draws them at random. The provider that runs the test and the remote proctoring is Selexi S.r.l.

There is also no detailed syllabus. What you have is [article 5 of the call](https://www.unisr.it/attachments/Call-for-Admission-International-MD-Program-AY-2026-2027-EU---ENG/26a5d91c-8469-4e24-b484-59ecd028e4e4/6a4285c1-507a-45e4-9d3b-582d348444fc.pdf), which sets out the structure of the test. That is the document to keep open while you plan your practice.

## The official UniSR simulator

UniSR sells its own simulator for this test. There is a free demo with 20 questions, and full simulations are paid. Check the current conditions on the [International MD admissions page](https://www.unisr.it/servizi/ammissioni/international-medical-doctor-program) before you buy.

The demo is a good way to see how the questions are worded and how the platform looks. It is not enough to train on its own, because 20 questions cannot tell you how you handle 60 questions against the clock.

## What a realistic simulation must reproduce

The 2026/27 call sets these rules, and a useful simulation should copy all of them:

| | International MD, 2026/27 |
|---|---|
| Questions | 60, all in English |
| Content | 36 on logical reasoning, problem solving and text comprehension; 24 on science (biology, chemistry, mathematics, physics) |
| Time | 80 minutes |
| Scoring | +1 correct, -0.25 wrong, 0 blank; maximum 60 |
| Reserve questions | 7, after the time is up; they do not count |
| Format | online, from home, two sessions; the better score counts |

The negative marking is the part most students underestimate. A wrong answer costs a quarter of a point, so guessing on every question you are unsure about can pull your score down. Practising with the same scoring teaches you when to answer and when to leave a question blank.

## How the TestBuddy simulation is built

The [TestBuddy simulation for the International MD](https://testbuddy.it/simulatore/san-raffaele-medicina-in-lingua-inglese) follows the official format: 60 questions, 80 minutes, +1 / -0.25 / 0.

There is one point where we had to make a choice, and we want to be clear about it. The call says 36 questions on logic, problem solving and comprehension and 24 on science, but it does not say how many questions each subject gets inside those two blocks. So our simulation uses this split:

| Subject | Questions in the TestBuddy simulation |
|---|--:|
| Logical reasoning | 30 |
| Text comprehension | 6 |
| Biology | 6 |
| Chemistry | 6 |
| Physics | 6 |
| Mathematics | 6 |

The totals match the call (36 + 24), but the subject-by-subject numbers are ours, not UniSR's. The real test may weigh the subjects differently, so do not skip any of them.

## Practising by subject

Because there is no official breakdown, the safest approach is to cover every area listed in article 5 and then let your results guide you:

- **Logic, problem solving and text comprehension.** With 36 questions out of 60, this block decides most of your score. Train it every week, not just in the final month.
- **Science.** 24 questions shared between biology, chemistry, mathematics and physics. Your weakest subject here is usually where the quickest points are.
- **Reading in English.** Every question is in English, including the science ones, so reading speed matters as much as content.

Start with untimed practice on each topic, then move on to timed blocks, and finally to full 80-minute simulations.

## Simulating exam conditions

In 2026/27 the test was taken online from home in two sessions, and the better of the two scores counted. That means your room, your computer and your concentration are part of the exam. When you run a full simulation:

- do it in one sitting, with a timer set to 80 minutes;
- keep your desk clear and your phone away;
- do not pause, and do not go back to check notes;
- after finishing, review every wrong answer and every blank, not just the score.

Two sessions are a real advantage: you can treat the first as a serious attempt and use what you learn to improve for the second.

## What score should you aim for?

There is no fixed pass mark. What matters is your position in the UniSR ranking compared with the number of places, and EU and non-EU candidates are ranked separately. In 2026/27 there were 86 EU places and 64 non-EU places. In the final rankings, position 86 on the EU list had 41.00 points and position 64 on the non-EU list had 48.75 points.

These are 2026/27 figures, not a promise for 2027/28. Use them as a reference and aim for a safety margin above them in your simulations.
