The International MD Program at Università Vita-Salute San Raffaele in Milan has its own admission test, with a fixed structure, negative marking and a tight time limit. In this guide we cover how the test is built, what to study when there is no detailed syllabus, where to start from scratch, how to organise 1, 3 or about 4 months of preparation, the most common mistakes and how to handle the last days.

A note on dates: the 2027/28 call is not out yet (as of 24 October 2026) and usually appears around mid-December. In 2026/27 the sessions were in February and March, so if you start now and the calendar stays similar, you have roughly three and a half months before a February session. Confirm the dates once the call is published.

## How the test is structured

The test is entirely in English and taken online from home, with remote proctoring.

- 60 multiple-choice questions
- 80 minutes
- 7 reserve questions after the time is up, not counted in the score
- two sessions, the better score counts

This is a different test from UniSR's [Medicine and Surgery test in Italian](https://testbuddy.it/test-universita-private/san-raffaele/guida-completa), so material made for that one will not match.

## Questions by block

| Block | Questions |
|---|--:|
| Logic, problem solving and reading comprehension | 36 |
| Science: biology, chemistry, mathematics and physics | 24 |
| Total | 60 |

The call does not split either block by subject. You know that 36 questions test reasoning and reading and 24 test science, but not how many are biology or physics. Plan your study around the two blocks, and do not trust any exact per-subject figure as official.

## Scoring system

| Answer | Points |
|---|--:|
| Correct | +1 |
| Wrong | -0.25 |
| Blank | 0 |

The maximum is 60 points. There is no pass mark: you need to rank within the places for your group. In 2026/27 there were 86 places for EU candidates and 64 for non-EU candidates, and in the final ranking the score at the last place of each quota (position 86 EU, position 64 non-EU) was 41.00 and 48.75. Those are last year's figures, but they show that a solid score, well above half marks, is what to aim for.

With a quarter-point penalty, random guessing works against you. Answer when you can rule out at least some options with confidence; otherwise, leaving it blank is a reasonable choice.

## What to study when there is no detailed syllabus

UniSR does not publish a topic list for this test: the reference is [article 5 of the call](https://www.unisr.it/attachments/Call-for-Admission-International-MD-Program-AY-2026-2027-EU---ENG/26a5d91c-8469-4e24-b484-59ecd028e4e4/6a4285c1-507a-45e4-9d3b-582d348444fc.pdf), which names the two blocks and the four science subjects. There are no official past papers either.

That leaves two things to work on.

**Logic, problem solving and reading comprehension.** This is the larger block, 36 questions out of 60. Work on:

- reading English passages quickly and answering questions on what they actually say
- logical deductions and arguments
- numerical problems and problem solving
- sequences and patterns
- working at speed without losing accuracy

**Science.** 24 questions shared between biology, chemistry, mathematics and physics. Cover the standard secondary school programme in each, in English, so that the terminology does not slow you down on the day.

## How to prepare

The priorities are:

- understanding the structure and the scoring
- training logic, problem solving and reading comprehension consistently, from the first week
- reviewing the four science subjects in English
- doing full timed tests of 60 questions in 80 minutes
- learning how to handle negative marking
- keeping track of your mistakes and the topics they come from

You can practise with the [TestBuddy simulator for the San Raffaele International MD test](https://testbuddy.it/simulatore/san-raffaele-medicina-in-lingua-inglese), which uses the same timing and scoring as the official one and shows where you are losing points.

## Where to start if you are beginning from scratch

- Learn the test structure and the scoring
- Do one timed practice test early, even if it goes badly, to see where you stand
- Start daily practice on logic and reading comprehension
- Pick up the science subjects one at a time, starting from the ones you are weakest in
- Keep practising questions from the start, not only at the end
- Use your mistakes to decide what to study next

The aim is not to study everything equally but to spend your time where the test gives points.

## 1-month study plan

With one month, focus on what moves your score fastest.

- Logic, problem solving and reading comprehension every day
- Quick review of the science topics you know least
- Timed sets of questions daily
- At least one full 80-minute test a week, then two in the last week
- Review every mistake

## 3-month study plan

### First month

- Learn the structure and do a first timed test
- Daily logic and reading comprehension practice
- Build foundations in biology and chemistry

### Second month

- Add mathematics and physics
- Increase the number of questions you do each day
- Start timed sessions

### Third month

- Full timed tests every week
- Work on pace: 1 minute and 20 seconds per question on average
- Strengthen weak topics
- Practise in conditions close to the real test

## About 4 months (late October to February)

If you start now and the first session again falls in February, you have a bit more room.

### Months 1 and 2

- Steady logic, problem solving and reading comprehension practice
- Biology and chemistry foundations
- Short timed sets from the start

### Month 3

- Mathematics and physics
- Mixed sets across all subjects
- First full timed tests

### Final weeks

- Full timed tests on a regular basis
- Error analysis and weak-topic review
- Final revision

Remember that there are two sessions. If the first one does not go well, you still have the second, and only the better score counts.

## What UniSR offers for practice

UniSR has not published past papers for this test. It sells its own online simulator, with a free 20-question demo and paid full simulations (prices on unisr.it). The demo is worth doing to see the official question style.

## Common preparation mistakes

- Using material made for the Italian-language San Raffaele test, which has a different structure
- Leaving logic and reading comprehension to the end, even though they are most of the test
- Dropping a science subject because there is no fixed number of questions for it
- Guessing at random despite the penalty
- Studying only theory and doing too few questions
- Not practising in English
- Never doing a full 80-minute test
- Ignoring the technical setup for an online test from home

## How to avoid them

- Organise your study around the two official blocks
- Practise a little every day rather than a lot once a week
- Review every wrong answer and note the topic
- Do full timed tests under real conditions
- Track your score over time
- Check your computer, webcam and connection well before the test

## The final week

The last week is for consolidation, not new topics.

- Full timed tests
- Review of your mistakes
- Logic and reading comprehension
- Key science formulas and definitions
- Pace and time management

## The day before and on the day

Because the test is taken online from home, technical preparation matters as much as revision. The day before, check:

- the instructions sent by UniSR and the provider, Selexi S.r.l.
- your identity document
- computer, webcam, microphone and internet connection
- a quiet room, with nothing on the desk that is not allowed

On the day, connect on time, follow the identification steps and stay in the room for the whole test. The exact rules are set out in the call and in the instructions you receive after registering.
